  1. Download Recommendation Form here.
  2. Request from any one of the following: school principal, guidance counselor, or class adviser (for Basic Education to College) / Immediate head in your current or last employment (for Graduate Studies/TCP) to accomplish it.
  3. Request the recommender to send the accomplished recommendation form to guidanceservices@spuqc.edu.ph with the applicant's full name and "Recommendation Form" as subject e.g. Maria Cruz - Recommendation Form.
